In this application you will be asked specific information about your identity such as your Full Name, Address, Age, Sex, Contact Number, Email Address and Learner Reference Number (LRN). All these information are needed for you to successfully register.
You are also required to upload pictures such as 2x2 Picture with name tag, Report Cards and Birth Certificate.
Under the terms of Data Privacy Act of 2012 (RA no. 10173), your data will be treated as highly confidential. This application shall only allow Southern Leyte State University to gather your personal data necessary for Southern Leyte State U Online Admission Application, record purposes and endorse qualifiers to respective department. Southern Leyte State U may also use your data for statistics, summaries, research and studies for development in the admission and academe.
You will need a unique email address to officially enter your application. Duplication of email address shall result to unsuccessful application.
